Inspector Notes
An unannounced renewal inspection was conducted on 6/21/2024. The inspector was on site from approximately 9:15 am-11:49 am. There were 8 children in care, ranging in age from 1 year to 5 years with one caregiver supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 5 children?s records and 1 caregiver record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations
3Evidence: The first day of attendance for Child #1 was documented in the record as 5/20/24. There was no physical examination in the record for Child #1. The provider acknowledged that documentation of a physical examination had not been obtained.
Evidence: In the children?s outdoor play area in the backyard, there was a rusted grill and compressor. There was a board on the back deck at the top of the steps that was warped and protruding which could cause a tripping hazard. There was peeling paint on the deck and on the trim around the back door. The children enter and exit the family day home through the back door. The provider acknowledged that the deck needs repairs and that the rusted equipment posed a hazard.
Evidence: In the corner of the deck leading to the back door there was an open box of mothballs and a can of RAID pesticide within reach of children. The children enter and exit the family day home through the back door. In the back yard near the children?s play equipment there was a full gasoline can and a container of Round Up weed killer. The provider acknowledged that these items were hazardous and needed to be stored away from the children.
