Inspector Notes
A renewal inspection was conducted on 5/15/23 from 11:15 am to 12:45 pm. There were 51 children present with 11 staff supervising. The children were observed engaged in various learning and play activities including but not limited to: outdoor play, circle time, puzzle and peg boards, and eating lunch. The lunch menu included macaroni and cheese, mixed salad and milk. The Pre-Kindergarten was observed preparing for their graduation ceremony. 5 staff records, 3 children's records, over the counter ointments and the emergency drill records were reviewed for compliance. The playground was inspected.

Violations
5Evidence: Staff # 1's first day of employment was 4/17/23. Staff # 1's TB screening was dated 12/28/21.
Evidence: The metal see saw on the playground has a pole in the center that is broken and rusty. The hole creates an injury hazard to the children.
Evidence: Child # 1's sunscreen was not labeled; and the center did not obtain written authorization for the sunscreen.
Evidence: Child # 2's diaper cream was not labeled with the name; and the center did not obtain written authorization for the cream.
Evidence: The written attendance for children in care did not document the departure of each child.
