Inspector Notes

An unannounced monitoring inspection was conducted from 3:00 pm - 5:00 pm. There were 15 children in care being supervised by 4 staff. The children were observed engaged in free play and eating snack. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-A
Based on review of three staff records it was determined that not all records contained documentation of a TB (tuberculosis) screening by the first day of employment. Evidence: The record for staff B (start date: 01/09/2023) did not have documentation of a TB screening.
Plan of Correction: Staff B will obtain a TB screening and submit the results to administration for their file.
Standard 8VAC20-780-270-A
Based on observation of the pre-k bathroom it was determined that not all areas were maintained in safe and operable condition. Evidence: 1. The wood paneling on the wall behind the toilet was observed to be splintering. 2. Chipping paint was observed on the bottom left of the bathroom vanity.
Plan of Correction: The wood paneling and vanity will be repaired.
Standard 8VAC20-780-280-B
Based on observation of the two's/three's room it was determined that not all hazardous substances were kept locked. Evidence: 1. Soap and bleach water was observed in an unlocked lower cabinet of the changing table. 2. Hand sanitizer was observed on a low toy shelf.
Plan of Correction: All hazardous substances will be kept locked.
Standard 8VAC20-780-290-A-3
Based on observation of the two's and three's classroom it was determined that not all outlets had protective covers. Evidence: The LI observed an electrical outlet uncovered near the entryway door.
Plan of Correction: All electrical outlets will have protective covers.
Standard 8VAC20-780-350-B-1
Based on observation and interviews it was determined that required ratios were not maintained for children birth through 16 months with one staff per 4 children. Evidence: The LI (Licensing Inspector) observed staff A caring for 5 children ages 11 months - 2 years.
Plan of Correction: Staff will follow the ratio of the youngest child in the classroom and will maintain a 1:4 ratio.
Standard 8VAC20-780-500-B
Based on observation of the infant room it was determined that not all diapering procedures were followed. Evidence: The LI observed debris on the changing table, indicating that the changing table was not cleaned and sanitized after use.
Plan of Correction: The changing table will be cleaned and sanitized after each use.
Standard 8VAC20-780-510-L
Based on observation of the infant room it was determined that not all medication was kept locked. Evidence: The medication for child A was observed in an unlocked refrigerator in the infant room.
Plan of Correction: Medication will be kept locked.
