During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on November 18, 2024. The on-site inspection began at 10:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above.There were 8 children present and 2 caregivers; the point total was 25points. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site. The inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
8Certification for CPR for the provider has been expired for over six weeks.
Certification for first aid for the provider has been expired for over six weeks.
The provider shall obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form.
Caregiver #1 has been in contact with children for over 18 months without obtaining a current report of tuberculosis screening.
The provider shall obtain for each caregiver a current Report of Tuberculosis (TB) Screening form, every two years from the date of the first screening .
The current TB screening for the provider has been expired for over 8 weeks.
Wood burning stoves and fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider.
The wood burning stove and associated chimney have not been inspected annually.
Annually, the provider shall review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
The emergency contact information for child #1 has not been reviewed for over 14 months.
The family day home had not obtained documentation that child #5 has been adequately immunized, who has been attending for over three weeks.
Child #2 who is under the age of two has not had updated immunization documentation for over 20 months.
Child #3 who is under the age of two has not had updated immunization documentation for over 23 months.
