Inspector Notes

An unannounced 60-day monitoring inspection was conducted on October 23 2023 between 7:00am and 8:00am. There were 12 children in the direct care of 3 staff members. Child were observed participating in a dance activity. A review of emergency drills, child records, and they physical space was observed. Sign in procedures were also reviewed. Based on record review, 1 of 2 child records reviewed did not have documentation that the children received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
Child #1 does not have an immunization record on file with the center.
Plan of Correction: Staff will work with families to obtain the immunization records from schools.
