Inspector Notes

An announced subsidy health and safety inspection (SHSI) was initiated and concluded on March 22, 2023. The licensing inspector was onsite from approximately 9:00am until 10:45am. There were three children in care one provider present. A monitoring inspection was also conducted on this date.

The information gathered during the inspection determined non-compliances with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Based on the review of four children?s records, the vendor did not ensure one child?s record contained the required information.

Evidence:
The record for #3 (first date of attendance 11/14/2022) did not contain the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: I will contact the parent and add the required information to the record.
