Inspector Notes

A monitoring inspection was conducted on 4/5/2023 from 11:40 am to 1:25 pm. Four children, ages 2 months to 2 years (total of 12 points) were in care with one caregiver. The Licensing Inspector reviewed 2 children?s records and other program records. The children were observed watching TV and playing.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Based on record review, the provider failed to ensure that child records contained some required information.

EVIDENCE:
1. The record for Child 1 did not contain the doctor?s phone number, insurance information, one of the emergency contacts didn?t have an address, and there was no authorization for the provider to obtain emergency care.

2. The record for Child 2 had no information regarding prior school, allergy information, health problems, an emergency contact did not have a complete address, and there was no authorization for the provider to obtain emergency care.

Based on record review, the provider failed to obtain proof of age and identity within seven business of the child?s first day of attendance.

EVIDENCE:
1. The record for Child 1 shows attendance began on 3/21/2023 and did not contain proof of age or identity.

2. The record for Child 2 shows attendance began on 3/22/2023 and did not contain proof of age or identity.

Based on record review, the provider failed to obtain immunization records showing children have been adequately immunized prior to attending the family day home.
