Inspector Notes

An unannounced renewal inspection was conducted today from 10:30am-11:30am. There were 5 children (infants-5yrs.) directly supervised by 2 staff. The physical plant, staff records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, puzzle time, tummy time and bottle feeding. Diapering and hand-washing procedures were also observed/ discussed. There was an supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice.

Based on record reviewed, required documentation of staff orientation was not completed.
Evidence: Staff #1 ( date of hire 6/26/2023) did not have documentation of orientation prior to staff member working alone with children and no later than seven days of the date assuming job responsibilities.
Plan of Correction: We completed the orientation. We will find and add to file.

Based on record review, required information in the child record was not complete.
Evidence:
Child A- record is missing documentation of viewing proof of the child's identity and age.
Plan of Correction: We will ask parent to bring in again to document in file.

Based on record review, required information was found to be incomplete.
Evidence: Staff #1's record is missing documentation of 2 references were checked before employment
Plan of Correction: We completed the references. We will find and add to file.

Based on record review, required parental agreements were not completed as required.

Child A- record is missing signed documentation by the first day of attendance of authorization for emergency medical treatment. Sick policies and communicable diseases,
