Inspector Notes

An unannounced monitoring inspection was conducted today from 11:30am-1:20pm. There were 13children (infants-5yrs.) directly supervised by 3 staff. The physical plant, staff records, children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating lunch. Diapering and hand-washing procedures were also observed. There was an ample supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Based on review of records, staff records did not contain documentation of completed background searches for a residence outside the state of Virginia within the past 5 years, in the required timeframes.

Evidence:
1. There were no results available for an out of state background search of criminal convictions, that were required to be on file prior to employment, for a residence outside of the state of Virginia ( Staff A).
2. There were no results available within 45 days of employment for a Child Abuse and Neglect search for a residence outside of Virginia in the past 5 years. (Staff A)
Plan of Correction: Reviewed proper documentation of out of state searches and chart on the www.childcareva.com website during inspection.
Standard 8VAC20-780-160-A
Based on record review, one staff record did not contain documentation of a negative Tuberculosis TB screen prior to employment and completed within the last 30 days.

Evidence:
Staff A's record did not contain a TB test completed before date of hire 11/27/2023.
Plan of Correction: Staff would not complete. Has made appointment today.

Based on observation, cot used for nap did not have linen consisting of a top and bottom cover.

Evidence:
1 child's cot did not have a bottom cover as required.
Plan of Correction: We will ask parent to bring in.

Based on record review and interview, documentation of 2 references were not completed before employment.

Evidence:
Staff A - Date of hire 11/27/2023 did not have 2 references documented before start date.
Staff B- Date of Hire 3/11/2024 did not have 2 references documented before start date.
Plan of Correction: We called and text references but we did not document in writing. We will document.
