Based on record review and interview, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunizations requirements once every six months for children under the age of two years. Repeat Violation.
Evidence:
1. The last documentation of additional immunizations for Child #3 (DOB 10/14/22) was dated 11/27/23.
2. The provider confirmed the last additional immunization documentation for Child #3 was dated 11/27/23.
Plan of Correction: I contacted the parents and they will bring the updated immunization record tomorrow. Going forward, I will add the children who need a 6-month immunization update to my calendar and then will remind the parents.
