During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on August 28, 2025. The on-site inspection began at 12:10pm and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 24. The inspector reviewed four children?s records and three caregivers/household members records on-site. This inspection included document review, tour of the home, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Child #3, who has been attending the family day home for over 60 days, did not have documentation of a physical examination on file.
Plan of Correction: I contacted the parent requesting the physical on or before 09/04/2025.
