Inspector Notes

An unannounced, on-site monitoring inspection was completed on, as a part of the licensure period. The on-site inspection began at 3:15 pm and ended at 4:15 pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and 2 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information.

The record for child #3 did not contain the father's work phone number.
The record for child #4 did not contain the mother's work phone number and the city and state for the two emergency contacts.

Standard 8VAC20-780-60-A-8
A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

There was no written care plan for child #5, diagnosed with a food allergy. Child #5 was in care during this inspection.
