3Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Of the five children?s records reviewed, three files are missing immunization records and have been enrolled at the center for over three months.
Plan of Correction: The center responded with the following: The center will obtain the three missing immunizations and place them in the children?s file. The center will ensure that all immunization records are in the children?s files prior to the first day of enrollment.
Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance. Three of the five files observed were missing documentation of a physical examination.
Plan of Correction: The center responded with the following: The center will obtain the missing physicals and will ensure that prior to enrollment that each child has a physical 30 days after the first day of attendance.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain work phone number of each parent who has custody, and phone number of two designated people to call in case of an emergency. Of the five files reviewed for children, four of the files were missing items required for enrollment.
Plan of Correction: The center responded with the following: The center will obtain all missing demographic items from the children?s parents at pick up today. The director will ensure that all information on the children?s demographic page is completely filled out.
