During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 1/31/2025 and completed on 3/14/2025 with a local agencies in response to a complaint received by the licensing office on 1/29/2025 relating to allegations in the area of care and protection. The inspector reviewed 1 child record and 1 staff record on site. This inspection included document review, a tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to proof of children's identity and age requirements, and immunizations for children and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/21/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #1, who was enrolled for over 2 weeks, did not have an immunization record on file.
Plan of Correction: This violation was corrected as of 2/3/2025. Moving forward the center shall obtain documentation that each student will have a record of their immunizations which is required by the state board of health before the child can attend the center.
Standard 8VAC20-780-40-G
The center shall meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia.

The center did not review and document proof of age and identity for child #1 who had been enrolled for over 2 weeks.
Plan of Correction: The violation was corrected on 2/3/2025. Moving forward, we will ensure that upon enrollment the center shall meet the proof of child identity and age requirements as stated in the code of Virginia.
