Enrollment checklists have been updated, and administrative staff have been retrained to confirm immunization records are received and reviewed before attendance begins.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
On the preschool playground, there were two slide structures with platforms that measured over one foot from the ground, and a see saw structure with moving beams. The three pieces of equipment were located on a grass covered surface and did not have the required resilient surfacing. Staff stated that the children had been using the equipment.

Plan of Correction: The center immediately discontinued use of the preschool playground equipment that lacked required resilient surfacing. The two slide structures and the see-saw were restricted from use until corrective measures could be implemented to ensure compliance with minimum safety standards. The center will install approved resilient surfacing that meets State Board of Health and playground safety standards under all playground equipment with moving parts or climbing apparatus, creating appropriate fall zones free of hazardous obstacles. Upon installation, the Director will conduct a safety inspection to verify compliance before allowing children to use the equipment. Staff have been informed and retrained that playground equipment may not be used unless proper surfacing is in place and approved for use.

For each infant, the center shall post a daily record which can be easily accessed by both the parent and the staff working with the child. The record shall contain a. The amount of time the infant slept; b. The amount of food consumed and the time; c. A description and time of bowel movements; d. Developmental milestones; and e. For infants, who are awake and cannot turn over by themselves, the amount of time spent on their stomachs.

Plan of Correction: The center has reinforced the requirement that a complete daily infant record be documented and posted daily for each infant and made accessible to both parents and staff. While infant staff sometimes verbally communicated daily information to parents, all required information is now being fully documented on the center's parent online app or written daily sheet. Effective immediately, infant staff have been retrained on daily documentation requirements including sleep times, feeding amounts and times, bowel movements, developmental milestones. and tummy-time for non-mobile infants. Supervisory staff will review daily infant records at the end of each day to ensure completion and accuracy. Verbal communication with parents will supplement-but not replace-required written or electronic documentation.
