Inspector Notes
An unannounced non-mandated inspection was conducted today between the hours of 11:00am-1:00pm. There were 4 children directly supervised by the provider and 1 caregiver. The home, 2 caregiver records, 5 children?s records, and evacuation drills were inspected. Children were observed during naptime. Areas of non-compliance are identified in the Violation Notice.

Violations
6Evidence:
1. Caregiver #1 began employment on 11/6/2023.
2. The record for Caregiver #1 did not contain the results of a central registry search.
3. A document in the record for Caregiver #1 indicated a request was sent on 3/4/2024. An additional document dated 3/6/2024 stated a portability request was denied and the individual's information was not found.
Evidence:
1. Caregiver #1 began employment on 11/6/2023.
2. The record for Caregiver #1 did not contain the name address and telephone number of a person to be notified in an emergency; and documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence:
1. The tuberculosis screening results in the record for Caregiver #1 were dated 6/30/2023, more than 30 days prior to the caregiver's date of hire (11/6/2023).
Evidence:
1. The record for Child #1 did not contain the name, address and telephone number for one of two designated persons to contact in case of an emergency if the parent cannot be reached;
Evidence:
1. Caregiver #2 stated topical products are used for Child #1,2,3,4 and 5.
2. Topical products were located in the family day home labeled with the children's names.
3. The provider did not obtain written parent authorization noting any known adverse reactions for the topical products for Child #1,2,3,4, and 5.
Evidence:
1. Documentation of immunizations in the record for Child #4 (start date 1/23/2024) were dated 1/29/2024, after the child's first day.
