Inspector Notes

An unannounced non-mandated inspection was conducted today between the hours of 11:00am-1:00pm. There were 4 children directly supervised by the provider and 1 caregiver. The home, 2 caregiver records, 5 children?s records, and evacuation drills were inspected. Children were observed during naptime. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-770-60-C-2
Based on record review, the provider did not obtain the results of a central registry search for 1 caregiver within 30 days of employment.

Evidence:
1. Caregiver #1 began employment on 11/6/2023.
2. The record for Caregiver #1 did not contain the results of a central registry search.
3. A document in the record for Caregiver #1 indicated a request was sent on 3/4/2024. An additional document dated 3/6/2024 stated a portability request was denied and the individual's information was not found.
Plan of Correction: The provider submitted a new Central Registry request for Caregiver #1.
Standard 8VAC20-800-120-B
Based on record review, 1 caregiver record did not contain all required information.

Evidence:
1. Caregiver #1 began employment on 11/6/2023.
2. The record for Caregiver #1 did not contain the name address and telephone number of a person to be notified in an emergency; and documentation that two or more references as to character and reputation as well as competency were checked before employment.
Plan of Correction: The provider will obtain the information from the caregiver.
Standard 8VAC20-800-170-B
Based on record review, a tuberculosis screening form was not completed within the last 30 days prior to employment for 1 caregiver.

Evidence:
1. The tuberculosis screening results in the record for Caregiver #1 were dated 6/30/2023, more than 30 days prior to the caregiver's date of hire (11/6/2023).
Plan of Correction: The caregiver will obtain a new tuberculosis screening by the end of the week.

Based on record review, 1 of 5 records reviewed did not contain all required information.

Evidence:
1. The record for Child #1 did not contain the name, address and telephone number for one of two designated persons to contact in case of an emergency if the parent cannot be reached;
Plan of Correction: The provider will request the information from the parents.
Standard 8VAC20-800-750-A
Based on caregiver interview and record review, the use of topical skin products did not meet all requirements.

Evidence:
1. Caregiver #2 stated topical products are used for Child #1,2,3,4 and 5.
2. Topical products were located in the family day home labeled with the children's names.
3. The provider did not obtain written parent authorization noting any known adverse reactions for the topical products for Child #1,2,3,4, and 5.
Plan of Correction: The provider will request written authorizations from the parents.
Standard 8VAC20-800-90-A
Based on record review, the provider did not obtain documentation a child has been adequately immunized before the child's first day for 1 of 5 children reviewed.

Evidence:
1. Documentation of immunizations in the record for Child #4 (start date 1/23/2024) were dated 1/29/2024, after the child's first day.
Plan of Correction: Provider will obtain documentation of immunizations prior to a child's first day.
