Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 09/17/2024, as a part of the licensure period. The on-site inspection began at 11:07am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 28. The inspector reviewed 3 children?s records and 3 caregiver records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service.

Evidence:
Caregiver #2 did not have central registry search results during the inspection and has been employed for 7 months. Caregiver #2 provided care as a substitute on the day of inspection.
Plan of Correction: A Central Registry Search request will be completed.
Standard 8VAC20-800-120-B
Providers must maintain a record for each caregiver. Caregiver records shall contain date of employment and documentation that two or more references as to character and reputation as well as competency were checked before employment.

Evidence:
Caregiver #2 had been employed for seven months. The record for Caregiver #2 did not contain the date of employment and documentation of two or more references.
Plan of Correction: Provider will request references and update date of hire in the record.

Providers are required to have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

Evidence:
The CPR certification for Caregiver #1 expired in July 2024.
Plan of Correction: Provider will call and schedule a class within the week.
