Inspector Notes

An unannounced Renewal monitoring inspection was conducted on July 21, 2023 from approximately 8:40am - 10:45 am . There were 6 children present at this time with 7 staff members. Additional staff was on site for administrative and support services. Children were observed learning to walk, tummy time, recogonition activity, playing on indoor slide and eating morning snack the contained yogurt and pears. Staff records and children records were reviewed during the Inspection. Licensing Inspector
reviewed indoor areas, supervision, activities, equipment, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Informatiopl gathered during the inspection determined noncompliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program

Based on record review and staff interview, the licensee did not ensure documentation of the screening was submitted at the time of employment and have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
Staff #1 employment date was 06/25/2023 and TB test was completed 06/30/2023.
Staff #2 employment date was 06/20/2023 and TB test was completed -6/22/2023.
Plan of Correction: Center Director will suggest for HR to have TB test done at the same time as background check paperwork is being completed.
