Inspector Notes

An unannounced renewal inspection was conducted at 11:50am. Observed 1 child totaling 2 pints with 1 caregiver. Ratios and supervision were in compliance. The child was napping during the inspection. Lunch was brought by the child. The daycare areas of the home were found to be clean, safeguarded appropriately and sufficiently supplied with toys and equipment for the children. Based on records reviewed, the licensee failed to ensure that 4 of 5 children's records included written information be given to the parents before the child's 1st day of attendance.

Evidence:

1. The Information for Parents form, signed by all parents, did not include written information regarding payments, the termination policy, and check-in/out procedures. Those areas of the form were left blank.
2. Child #2, #3, #4, and #5 began attending between 6/1/23-7/10/23.
Plan of Correction: The provider said she will make a master form and write all of the required general information onto the form, make copies, add the personal information and have parents sign it.
