Inspector Notes

An unannounced monitoring inspection was conducted at 10:15am. Observed 3 children totaling 4 points with 1 caregiver. An adult household member was also present. Ratios and supervision were in compliance. Children were observed playing with doing school work, reading and playing with toys. The daycare was sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Based on records reviewed, the licensee has not kept a written record of children in attendance each day. Evidence: The last date of attendance records was dated 3/22/24.

Based on records reviewed, 1 of 3 children's records did not include immunizations every 6 months until the age of 2 years old. Evidence: The immunizations on-file for Child #1 were dated August 2022 when the child was 16 months of age.
Plan of Correction: I show you mom's text. She has appoint schedule on 4/30 since its her immunizations date to get shot [sic]

Based on observations made and interview conducted, there was not a menu for the current one-week period. Evidence: There was not a menu posted for viewing and the licensee stated that a menu had not been written.
