Inspector Notes

A complaint was received in the licensing office on 9/20/2023 regarding staff training/qualifications and notification of diagnosed illnesses. On 9/20/2023 an on-site complaint investigation was conducted, as well as a non-mandated monitoring inspeciton. Interviews were conducted and observations were made. At the time of this investigation, the preponderance of evidence supports the allegation, and the complaint is deemed valid. Violations were cited and can be found on the Violation Notice. Standard 22.1-289.035-B-2
Based on a review of staff records, 1 out of 4 staff did not submit to fingerprinting prior to the
first date of employment.

Evidence:
1. Staff #3 (doh: 7/10/2023) did not have fingerprints results on file at the time of the inspection.
Plan of Correction: We will have the staff member do the required background check immediately and will do them on time for each new employee moving forward.
Standard 22.1-289.035-B-4
Based on a review of staff records, the licensee did not obtain a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry
or equivalent registry from any state in which the individual has resided in the preceding five
years for all staff.

Evidence:
1. Staff #1 (doh: 9/12/2022) did not have a criminal history record information check
completed for Illinois on file at the time of the inspection.
2. Staff #2 (doh: 1/7/2023) did not have the results of a search of the child abuse and neglect registry or equivalent registry for Maryland at the time of the inspection.
3. Staff #3 (doh: 7/10/2023) did not have the results of a search of the child abuse and neglect registry or equivalent registry for Maryland at the time of the inspection.
Plan of Correction: We will send the required out-of-state background checks out right away.
Standard 8VAC20-780-490-C
Based on a review of documents and staff interview, when children at the center have been exposed to a communicable disease listed in the Department of Health's current communicable disease chart, the parents were not notified within 24 hours or the next business day of the center's having been informed.

Evidence:
1. The Center Director stated that since 8/31/2023 " There have been 4 cases of Conjunctivitis in our school". A written illness notification was sent to notify families on 8/31/2023 regarding one diagnosed case of Conjunctivitis. Parents were not notified within 24 hours or the next business day after the other 3 cases of conjunctivitis were diagnosed and reported to the facility.
Plan of Correction: Moving forward, we will be sure to notify parents of each diagnosed illness within the timeframe required.
