Inspector Notes

On 2/1/2024 an unannounced investigation was conducted regarding a self-reported incident regarding supervision of children. This visit was conducted jointly with the Child Protective Services agency between 10:00am-11:45am. Interviews were conducted, records were reviewed and observations were made. After carefully interviewing all involved parties, the investigation is unfounded. There were other violations cited during this visit. These violations are listed on the Violation Notice. Based on a review of staff records, all staff did not have a central registry finding within 30 days of employment.

Evidence:
1. Staff #1 (doh: 12/29/2023) did not have a central registry finding on file at the time of the
inspection, which was past the employee's 30 days of hire.
Plan of Correction: We have received the employee's central registry results after following up.
