Inspector Notes

An unannounced inspection was initiated on 11/7/2024 and completed on 12/5/2024 in response to a complaint received by the licensing office on 11/4/2024 relating to allegations of consistency of care, ratios, and hazardous cleaning agents. The inspector reviewed 1 child record and 1 staff record on-site on 11/7/2024. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation did not support the allegations; therefore, the complaint is determined to be not valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to background checks and are documented on the violation notice issued to the program.

**Repeat Violation**
The Child day center is required to obtain a copy of the results of a criminal history record
information check, a sex offender registry check, and a search of the child abuse and
neglect registry or equivalent registry from any state in which an individual (employee) has
resided in the preceding five years.

Staff #1 did not have results of an out-of-state Central registry check and sex offender registry check for a state in which the employee has lived in the preceding 5 years and has been employed for 2 months.
Plan of Correction: We initiated the process for the out-of-state background checks that were missing.
