During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/6/2025. The on-site inspection began at 11:00am and ended at 2:35pm. The inspector reviewed compliance in the areas listed above. There were 91 children present and 16 staff. The inspector reviewed 7 children?s records and 6 staff records on-site on 3/6/2025.This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/7/2025. A POC submitted after this date will not appear on the public website.
Violations
7The Child day center is required to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which an employee has resided in the preceding five years.
Staff #3 and Staff #7 did not have documentation of the results of an out-of-state sex offender check for a state in which the employees have resided in the preceding five years.
Staff #6 did not have documentation of the results of an out-of-state criminal history record information check for one state, a sex offender registry check for two states, and a search of the child abuse and neglect registry or equivalent registry from two states in which the employee has resided in the preceding five years.
As of Monday March 10, 2025 Celebree School of Alexandria has submitted requests for all missing out-of-state background checks for Staff #3, Staff #6, and Staff #7.
Documentation of submission and processing status will be maintained in each employee?s personnel file.
Staff members missing required documentation will not be left alone with children until all required checks are received and verified.
Ongoing Compliance Monitoring:
Using the tracking system/ spreadsheet with fidelity to log all staff background check requests, dates of submission, expected completion dates, and receipt confirmations.
Child #2 did not have documentation of a physical examination on file.
Three children under two years of age were observed using pillows during the inspection.
There was a prescription medication observed in a plastic bag with no child's name, no name of the medication, no dosage amount and no time to be given labeled on the bag.
Prepared infant formula is required to be refrigerated, dated and labeled with the child's name.
There were 2 bottles observed in the refrigerator with no name/date for the day and 5 bottles observed to have a name, but no date for the day.
Immediate labeling and dating of all bottles in the infant room.
Implementation of a daily bottle check by infant room teachers and administrative staff to ensure compliance.
Staff training on proper labeling and storage procedures for infant formula.
Notification sent to all families reminding them of the requirement to label and date bottles before drop-off.
Parents will be provided with labels and markers at the front desk to support compliance.
Random weekly inspections to ensure ongoing adherence to this policy.
Child #3 did not have documentation on file of an annual update of emergency information.
Child #2 did not have documentation on file of proof of identification being verified.
Staff records are required to be kept for each staff person including documentation of 2 references of character and reputation taken prior to hire.
Staff #4, Staff #5 and Staff #7 did not have documentation of 2 out of 2 required references during the inspection.
As of Friday, March 7, 2025 Celebree School of Alexandria has contacted the listed references for Staff #4, Staff #5, and Staff #7 and is actively obtaining the required documentation.
All reference checks will be completed and added to staff records within 24 hours.
Moving forward, staff missing required references will not be scheduled until documentation is received
Staff Training & Policy Reinforcement:
All hiring managers and administrative staff will receive training on Virginia?s hiring documentation requirements, including the necessity of obtaining two character references before onboarding.
The hiring policy will be updated to require written verification that reference checks are completed before new hires begin working.
