Inspector Notes
An unannounced inspection was initiated on 3/6/2025 and completed on 3/6/2025 in response to a complaint received by the licensing office on 3/4/2025 relating to allegations of behavioral guidance concerns.The inspector reviewed 1 staff record on- site on 3/6/2025. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to staff records and are documented on the violation notice issued to the program.
Violations
1staff member is required to submit documentation of a negative tuberculosis screening.
Staff #1 did have documentation of a negative tuberculosis screening by day one of employment, but no more than 30 days prior.
