During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site non-mandated monitoring inspection was initiated and completed on 8/12/2025 in response to a self-reported incident regarding child supervision. The on-site inspection began at 12:40pm and ended at 2:10pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 1 child's record and 3 staff records on-site on 8/12/2025. This inspection included: document review, tour of the facility, interviews, and observations. Though not a part of the original self report, violations that were not reported were found related to staff and child records and are documented on the violation notice issued to the program.

Each child shall have a physical examination by or under the direction of a physician within 30 days of their enrollment date.

Child #1 did not have documentation of a physical examination on file at the time of the inspection. Child #1's enrollment at the facility began 10 months ago.
Plan of Correction: We will reach out and ask the child's parents for this information.
Standard 8VAC20-780-160-A
REPEAT VIOLATION:
Each staff member is required to submit documentation of a negative tuberculosis screening.

Staff #1 did not have documentation of a negative TB screening on file at the time of hire. Documentation was not received until the second day of employment.
Plan of Correction: We will make sure we receive the TB screening prior to hire moving forward.
