An unannounced inspection was initiated and completed on 9/18/2025 in response to a complaint received by the licensing office on 9/17/2025 relating to allegations of inappropriate behavior guidance and forbidden actions. The inspector reviewed 1 staff record on-site on 9/18/2025. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
