Inspector Notes

An unannounced inspection was initiated and completed on 9/18/2025 in response to a complaint received by the licensing office on 9/15/2025 relating to allegations of supervision and ratio concerns.The inspector reviewed 1 staff record on-site on 9/18/2025. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to background checks and staff records and are documented on the violation notice issued to the program.
