Standard 22.1-289.035-B-4
The child day center is required to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Staff #4 did not have documentation on file of the results of a search of the child abuse and neglect registry for a state in which the staff member resided in the preceding five years.
Plan of Correction: The background check was sent in November, but we will follow-up to find out about the results and send it out again if needed.
Standard 8VAC20-780-160-A
Each staff member is required to submit documentation of a negative tuberculosis screening.

Staff #4 did not have documentation of a negative tuberculosis screening on file at the time of the inspection.
Plan of Correction: We will have the staff member get a TB screening as soon as possible.
Standard 8VAC20-780-245-J-3
Any child for whom emergency medications have been prescribed shall always be in the
care of a staff member or independent contractor who meets the requirements in
subdivision 1 of this section.

Child #1 had an emergency medication prescribed, however, there were no staff onsite
with the required training to be able to administer the emergency medication should
the need arise.
Plan of Correction: We will have the training completed by 12/26 or sooner by 1-2 staff members.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

There were 5 bottles in the refrigerator in one of the infant classrooms with no date on them. 1 out of the 5 bottles with no date was also missing a name.
Plan of Correction: We spoke to staff about labeling all bottles. Management will check that the bottles are labeled at 10am during the daily walk through as well.

A written care plan is required for each child with a diagnosed food allergy, to include
instructions from a physician regarding the food to which the child is allergic and the steps
to be taken in the event of a suspected or confirmed allergic reaction.

Child #1 has a diagnosed food allergy on file at the facility, however, there was no written care plan on file.
Plan of Correction: We will give the form to the parent to get filled out today.
