During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on January 10, 2025 and completed on January 14, 2025. The inspection started at 10:30 a.m. and concluded at 11:30 a.m. There were two children present and one caregiver; the point total was three. The inspector reviewed three children?s records, one caregiver and three household members records on-site and electronically on January 14, 2025.

This inspection included document review and tour of the facility

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Caregivers shall undergo TB screenings at least every two years from the date of the initial screening, or more frequently if recommended by a physician.

Caregiver #1's most recent TB screening results were dated 7/8/2022. The TB screening update was more than six months overdue.
