Inspector Notes

An unannounced renewal inspection was conducted today from 9:00am-10:00am with the provider on site. There were 8 children (25 points) present with 3 caregivers. A sample of 3 children's files and 3 caregiver/household files were reviewed. The children were engaged in the following: washing hands for breakfast, eating, circle time, diaper changes and playing with toys. The physical space, attendance records, drill record, menu and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice. Repeat violations were found and are identified within this report.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard 8VAC20-800-100-A
Repeat Violation
Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1's file (start date 8/21/2023) did not have a copy of the results of a health exam.
Plan of Correction: Contacted child's parent for physical examination.
Standard 8VAC20-800-210-A
Based on record review and interview, two caregivers did not obtain a minimum of sixteen clock hours of training annually.
Evidence:
1. Caregiver #2 (start date 2/27/2023) and Caregiver #3 (start date 2/27/2023) did not have 16 hours of training for the period 2/2023 through 2/2024.
Plan of Correction: Both caregivers will complete training for the upcoming period. From 7/15/2024 for the upcoming period.
Standard 8VAC20-800-60-B
Based on record review, a child's record did not contain all required information.
Evidence:
1. Child #1's file (start date 8/21/2023) was missing the health insurance policy number and results of the health exam.
Plan of Correction: Contacted child's parent for policy number.
Standard 8VAC20-800-680-B
Based on observation, caregivers did not ensure that children's hands are washed with liquid soap and warm running water after diapering.
Evidence:
1. Caregiver #3 was observed changing an infant's diaper and then put the infant on the floor to play and did not wash the infants hands after diapering.
Plan of Correction: Caregiver will wash infants hands after changing.
Standard 8VAC20-800-730-D
Based on observation, the provider did not keep a medication locked.
Evidence:
1. A Bottle of Excedrin was on a small table near the telephone in the small room. The door was not locked and it was not kept in a locked place using a safe locking method that prevents access by children.
Plan of Correction: Medication will be stored away properly.
