During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 12/11/2024. The on-site inspection began at 8:43am and ended at 9:22am. The inspector reviewed compliance in the areas listed above. There were six children present and three caregivers; the point total was 17. The inspector reviewed four children?s records and five caregiver/household members records on-site on 12/11/2024.
This inspection included document review, tour of the home and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 12/18/2024. A POC submitted after this date will not appear on the public website.
Violations
5The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
There were no physical exam records available for Child #1, who had been in care for over one year and three months.
An updated TB screening for Caregiver #1 (overdue by four weeks) , Caregiver #2 (overdue by three months) and Caregiver #3 (overdue by one month) , and Household Members #1 (overdue by two weeks) and Household Member #2 (overdue by one week) was not completed.
Caregivers shall obtain a minimum of 16 clock hours of training annually.
Caregivers (#2 and #3) did not obtain 16 hours of training and have been employed for one year and 9 months.
Children's records are required to contain the medical insurance policy number and parent's employment phone numbers.
The record for Child #1 was missing the medical insurance policy number. The child was present during the inspection and had been enrolled for one year and three months.
The record for Child #3 was missing both of the parent's employment phone numbers. The child was present during the inspection and had been in care for 9 months.
