Inspector Notes
An unannounced monitoring inspection was conducted on 05/16/2024. There were 63 children, ages 6 months - 5 years, 13 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during self-directed activities, teacher directed activities, and during lunch. A total of 6 children?s records, 6 staff records and 4 medications were reviewed. The inspector discussed the following with the provider: diapering and sanitizing. The inspector arrived for the inspection at approximately 9:20 am and departed at approximately 11:55 am.
The information gathered during the inspection determined non- compliance with applicable standards or law and violations were documented on the violation notice to the provider.
Violations
3Evidence: The record for Staff 2 contained evidence that Staff 2 had resided outside of the state of Virginia within the preceding five years. The record for Staff 2 contained a documented hire date of 02/22/2024. There was not a child abuse and neglect registry check from all of the states in which the individual had resided in the preceding five years available for review during the 05/16/2024 inspection. Staff 2 was observed working during the inspection.
Evidence:
1. Staff 1 was observed pouring and serving a child a cup of milk. Staff 1 did not wash their hands before preparing and serving the beverage. The child did not wash their hands before drinking the cup of milk.
2. Staff 7 was observed wiping a child?s nose. Staff 7 did not wash their hands after coming into contact with body fluids.
Based on observation, the center failed to ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. There were 5 children observed in the Step Up Room; the attendance had not been taken. The attendance record did not document the arrival of each child as it occurred.
2. There were 9 children observed in the Infant Room; there were 11 children marked present on the attendance record. The attendance record did not document the departure of each child as it occurred.
3. There were 8 children observed in the Older Two?s Room; there were 9 children marked present on the attendance record. The attendance record did not document the departure of each child as it occurred.
4. There were 10 children observed in the 3-5 Year Old Room; there were 11 children marked present on the attendance record. The attendance record did not document the departure of each child as it occurred.
5. There were 8 children observed in the Younger 2?s Room; there were 10 children marked present on the attendance record. The attendance record did not document the departure of each child as it occurred.
6. There were 10 children observed in the 16-24 Month Room; there were 14 children marked present on the attendance record. The attendance record did not document the departure of each child as it occurred.
7. There were 13 children observed in the 3-5 Year Old Room; there were 9 children marked present on the attendance record. The attendance record did not document the arrival of each child as it occurred.
