Inspector Notes

An unannounced, onsite monitoring inspection was initiated on 11/04/2024 and completed on 11/07/2024. The on-site inspection began at 11:30 am and ended at 2:00 pm. The inspector reviewed compliance in the areas listed above. There were 58 children present and 14 staff. The inspector reviewed 6 children?s records and 6 staff records.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child 5, who has been attending for 3 months, did not have documentation of an immunization record on file.
Plan of Correction: Will get from parents.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The immunization records for Child 2 (2 years), who has been attending for 15 months, were not updated once every six months. The immunization records for Child 4 (17 months), who has been attending for 15 months, were not updated once every six months.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained within 30 days after the first day of attendance.

There was no physical examination record for Child 6, who has been enrolled for 3 months.
Plan of Correction: Will get from parents.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

The TB screening for Staff 3 was dated 7 weeks prior to the date of employment.
Plan of Correction: Will make sure all of the TB screenings have been done within 30 days.

The center shall implement a monthly practice evacuation drill.

Evacuation drills were not practiced in January 2024, February 2024, April 2024 or October 2024.
Plan of Correction: Will do monthly drills and make sure to document them.
Standard 8VAC20-780-60-A
The center shall maintain a separate record for each child enrolled that contains all of the required information.

Staff 7 was unable to locate the record for Child 5, who has been attending for 3 months.
Plan of Correction: If the form cannot be found, the parent will be asked to complete a new form.
