An unannounced monitoring inspection was initiated and completed on 1/27/25 in response to a self-report received by the licensing office on 1/19/25 relating to forbidden actions. The inspector reviewed 2 children?s records and 2 caregiver's records on-site on 1/27/25. This inspection included document review, tour of the facility, interviews, and observations.
