Inspector Notes
An unannounced monitoring inspection was conducted on 12/20/2023 from 9:10 am to 10:35 am There were 7 children (19 points) supervised by 3 direct caregivers. The home, 3 caregiver records, 5 children?s records, 1 household member record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed eating breakfast and engaging freely. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Thank you for your cooperation during this inspection.

1. There was no documentation of a physical examination completed by or under the direction of a physician before or within 30 days of Child #5's first day of attendance.
2. The start date for Child #5 is 10/16/2023.
Evidence: The was no documentation of orientation for Caregiver #2.
Evidence:
1. There was no documentation of written parental authorizations for the universal diaper cream used in the family day home.
2. There was no documentation of written parental authorizations for 6 out of 6 topical ointment found in the 6 out of 6 individual student cubbies.
Evidence: 6 out of 6 topical ointments were found in individual children's cubbies located on the bottom of a cubby shelf.
