Inspector Notes
An unannounced monitoring inspection was conducted 5/6/2024 from 11:15am-12:15pm. There were 6 children(5 totaling 14 points), including the provider's child directly supervised by 2 caregivers. The home, 2 caregiver records, 2 children?s records, pet record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, eating snack and napping. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

5Evidence:
1. Caregiver #2's record did not have name address and telephone number of a person to be notified in an emergency;
2. Caregiver #2's record did not have verification of age.
Evidence: Caregiver #1's CPR certification expired 2023.
Evidence: Caregiver #1's first aid certification expired 2023.
Evidence:
1. There was no documentation of orientation in Caregiver #2's file.
2. The provider stated they were unaware that part time/assistant caregivers required an orientation.
Evidence: Child #2's file did not contain a second designated persons to contact in case of an emergency if the parent cannot be reached.
