Inspector Notes
An unannounced renewal inspection was conducted 7/10/2024 from 10:10am-11:20am. There were 5 children in care and caregivers 3 children, totaling 16 points, directly supervised by 2 caregivers. The home, 3 caregiver records, 3 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed eating snack, washing hands, napping and playing freely. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Violations
5Based on record review and interview, the provider does not have current certification in cardiopulmonary resuscitation issued within the past two years.
Evidence: Caregiver #1 CPR certification expired 2023.
Evidence: No training hours, other than CPR and First Aid were documented for Caregivers #1, #2 or #3 between 7/2023-2024.
Evidence: The provider did not have documentation of Caregivers #2 and #3 education.
Based on record review, the provider did not maintain a child's record that contained all required information.
Evidence: Child #3's record did not contain name, address and telephone number of a second designated persons to contact in case of an emergency if the parent cannot be reached.
for a child who is not exempt from the immunization requirements, the family day home did obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements, once every six months for children under the age of two years.
