During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 01/22/2026, as a part of the licensure period. The on-site inspection began at 2:11pm and ended at 2:55pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 23. The inspector reviewed 4 children?s records and 2 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The findings of the most recent inspection shall be posted in a conspicuous place on the licensed premises.

The home's most recent inspection was conducted on July 11, 2025. The findings from this inspection were not posted.
Plan of Correction: An Acknowledgement form for 1-22-2026 inspection was posted. Provider will post final inspection notice.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

Emergency drills were not conducted in November or December 2025. Provider stated that due to Holidays, they forgot to conduct.
Plan of Correction: Provider will ensure drills are conducted monthly.
