Inspector Notes

An unannounced complaint inspection was initiated on Thursday, May 2, 2024, and concluded on Tuesday, May 28, 2024. The complaint was received by the department on April 17, 2024, and was regarding the alleged non-compliance in the areas of supervision; injury reporting; and administration. On May 2, the inspector was on-site from 11:10am until approximately 12:00pm. Interviews with staff were completed, observations were made, and documentation was reviewed. Collateral interviews were conducted on May 2, 2024. On May 28, the inspector was on-site conducting additional observations and interviews from 10:10am until approximately 12:30pm.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint was determined to be not valid. There were no violations resulting from this inspection.
