Inspector Notes

An unannounced complaint inspection was initiated on Tuesday, May 14, 2024, and concluded on Tuesday, May 28, 2024. The complaint was received by the department on May 8, 2024, and was regarding the alleged non-compliance in the areas of supervision; injury reporting; and
administration. On May 28, the inspector was on-site from 10:10am until approximately 12:30pm. Interviews with staff were completed, observations were made, and documentation was reviewed. Collateral interviews were conducted on May 14, 2024.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint was determined to be not valid. There were no violations resulting from this inspection.
