Inspector Notes

An unannounced monitoring visit was conducted. Three children were in care for eight points with one caregiver. Observed the youngest sleeping in the crib, free play and transition for the other two to rest time. Home has 2 rooms for daycare as well as the bathroom and a large backyard area for outside play. The children bring all food from home. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.. Based on record review, a child is missing a physical exam record within 30 days of enrollment.
Evidence:
Child #2 enrolled 1/2023 and does not have a physical record on file.
Plan of Correction: Will request information from parent.

Based on observation, a container of cleaning wipes was not inaccessible to children.
Evidence:
Clorox wipes were in the bathroom and not stored out of reach.
Plan of Correction: Moved during the inspection.

Based on provider statement and a review of the attendance records, attendance was not taken every day.
Evidence:
No attendance record was maintained January 3rd-January 22nd of children in care during those days.
Plan of Correction: Moving forward attendance will be taken each day to ensure an account of each child in care,

Based on record review, children's records are incomplete.
Evidence:
1. Child #1 is missing 2 emergency contacts and addresses as well as work information for parents.
2. Child #2 is missing documentation of an annual update.
Plan of Correction: Request information from parents.

Based on record review, an updated immunization record for a child under two years of age is not on-site.

Child #3 is missing an updated immunization record-last update was 10/23.
