During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 7/03/2025. The on-site inspection began at 8:45am and ended at 10:10am. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was 8. The inspector reviewed five children?s records and four caregiver/household members records on-site on 7/03/2025.
This inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/10/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The license and any other documents required shall be posted in a conspicuous place on the licensed premises.

The findings from the most recent inspection, dated 1/09/2025, were not posted in a conspicuous place on the premises. The inspection report was underneath the license.

Standard 8VAC20-800-110-A
The current form approved by the Virginia Department of Health or a physician's
form shall be used to record immunizations received and the results of the required
physical examination. Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the
physician's designee, or an official of a local health department.

Child #4 enrolled for 15 months did not have immunizations or physical examination on the Virginia Department of Health or physician's form and it was not signed. The provider had a print out of the child's medical history only.
Plan of Correction: We are working with the parent of child #4 to obtain the
immunization and physical examination records on the
Virginia Department of Health or physician?s form, with all required
signatures. We will remove the printouts once we have received
the records in the approved format.
Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

The provider had 10 hours of training for the time period 8/2023 through 8/2024.
Plan of Correction: have scheduled continuous training sessions in the Child Care

Standard 8VAC20-800-240-A
'Areas and furnishings of the family day home, inside and outside, shall be
maintained in a clean, safe, and operable condition.

The shed in the backyard had a wooden door that was splintered, had deteriorating wood, and had chipped paint, accessible to children, that was a hazard to the children in care.
Plan of Correction: We are working with a contractor to replace the splintered door with
a new one.

Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

One unused electrical outlet in the play room was not covered with a child resistant protective cover and the provider had children present under five years of age.

Children's records are required to contain all of the required information.

The record for Child #4, enrolled for 15 months, was missing the following required
components: written authorization for emergency medical care should an
emergency occur and the parent cannot be located immediately unless the parent
presents a written objection to provision of medical treatment on religious or other
grounds.

The parents of Children #1 and #2, were not provided in writing the following required

and response plan. Children had been in care for 11 months.

The provider did not have the responsible person for emergency backup care posted. It only listed the telephone number and not the name of the person.

All perishable foods and drinks used for children in care, except when being prepared and served, shall be kept in the refrigerator.

Two children had food from home that was perishable and was not kept in the refrigerator.
Plan of Correction: We will keep all perishable foods and beverages in the refrigerator
