Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on January 22, 2025 and completed on January 22, 2025. The inspection started at 12:30pm and concluded at 1:01pm. There were zero children present and one caregiver; the point total was zero. The inspector reviewed zero children?s records and two caregiver/household member(s) records.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The provider is required to have applicants and household members undergo a background check every five years.

The most recent national criminal background check for applicant #1 was overdue by nineteen days and household member #1's was overdue by sixteen days.
Plan of Correction: Have already submitted the request for the repeat background checks on January 21st 2025.

Caregivers are to complete the department's health and safety update course annually.
