Inspector Notes

An announced initial inspection was conducted on 2/23/2023 from 10:00am to 11:50am. There were no children currently enrolled in the program. The physical plant, 2 staff records, emergency supplies, and policies were inspected. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on observation, the facility did not ensure that the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: observed that there was a hole in the tile floor in the infant room, near the high chairs, and another hole along the side trim, along the floor.
