Inspector Notes

An unannounced monitoring inspection was initiated on 10/07/2024 and completed on 12/03/2024 in response to a self-report received by the licensing office on 10/04/2024 relating to special services and programs. The inspector reviewed one child?s record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/18/2024. A POC submitted after this date will not appear on the public website.

Formula or breast milk shall not remain unrefrigerated for more than two hours and may not be reheated.

It was determined that Child #1 was given a warmed bottle with breast milk at approximately 4:20pm on 10/03/24. Child #1 did not consume the bottle right away. The parent arrived for the child, staff reported that they went back into the classroom and put the bottle back in the warmer. The bottle was reheated approximately 30 minutes after the first time the bottle was warmed.
Plan of Correction: Per the Center: "On 12/03/24, staff was retrained on proper safety on handling formula & breast milk. To ensure moving forward proper handling steps are followed, supervisor will do monitor checks with staff."
