Inspector Notes
An unannounced, on-site renewal inspection was conducted on 8/6/24. The on-site inspection began at 9:45am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were seven children present and two caregivers; the point total was 15. The inspector reviewed four children?s records and three staff records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/16/24. A POC submitted after this date will not appear on the public website.

Based on observation, the provider did not ensure that the license and any other documents required by the Superintendent were posted in a conspicuous place on the licensed premises. Evidence: The most recent inspection notice and violation summary are required to be posted in a conspicuous place on the premises and were not posted on 8/6/24.

Based on review of four child records, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance for one child. Evidence: There was no physical on record for Child D, with a start date of 6/3/24.

Based on review of documentation and interview, the provider did not obtain a parent?s written authorization for a long-term emergency medication. Evidence: Two emergency medications were observed on-site for Child B. There was no documentation on record for Child B of a parent?s written authorization to administer the emergency medications.
