During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 8/19/25 and completed on 8/22/25. The on-site inspection began at 11:10am and ended at 12:50pm. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 24. The inspector reviewed four children?s records, two caregiver records, three household member records, and three pet records on-site and electronically. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/29/25. A POC submitted after this date will not appear on the public website.
Violations
7The record for Caregiver B contained documentation that the caregiver had lived outside of Virginia within the past five years, however, there was no documentation that the provider had requested a sex offender check or central registry check from the state where the caregiver previously resided.
Central registry results were past due for Household Member C by more than one month.
The record for Caregiver B did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment. Documentation is required to include dates of contact, names of persons contacted, results, and the signature of person making the call.
A subsequent TB screening was past due for Household Member A by more than seven months.
On 8/19/25, Child A, nine-months-old, was observed napping in a play pen. A caregiver confirmed that was her designated sleep space.
On 8/19/25, a Caregiver was observed diapering multiple children without washing their own hands with liquid soap and warm running water.
On 8/19/25, a Caregiver was observed diapering multiple children without ensuring the children?s hands were washed with liquid soap and running water.
