A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Caregiver #1 and Household Member #1 were not completed. The TB for Caregiver #1 was overdue by more than 12 months and the TB for Household Member #1 was overdue by 10 months.
Plan of Correction: The repeat TBs will be obtained as soon as possible.

Children's records are required to contain written authorization for emergency medical care should an emergency occur, and the parent cannot be located immediately.

The records of Child #1 and Child #3 were missing the written authorization for emergency medical care. The children were present during the inspection and have been in care for two and three months.

Records of emergency evacuation and shelter-in-place drills shall be maintained for one year.

The emergency evacuation drill conducted in November 2025 was not documented.
