Inspector Notes

An unannounced non-mandated inspection, as required by the Conditional License, was conducted on 02/27/2023 for Ted and Ted?s Tiny Tots. There were 13 children, ages 6months - 4 years, 3 staff and 1 volunteer present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 4 children?s records and 1 staff record were reviewed. The children were observed during center time, creative play, clean up and lunch. The inspector arrived for the inspection at 10:20am and departed the center at 11:49am.

Discussion on standards related to special feeding needs. Discussion on standards related to restroom areas and furnishings.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on review of four children?s records, the center failed to ensure that all immunization records were obtained before the child?s first date of attendance.

Evidence: Child #2 first date of attendance was 1/9/2023 and on the day of the inspection did not have the required immunization record.
Plan of Correction: Provider will obtain immunization record.
Standard 8VAC20-780-140-A
Based on the review of four children?s records, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

Evidence: Child #2 first date of attendance was 1/9/2023 and on the day of the inspection did not have the required physical examination record.
Plan of Correction: Provider will obtain physical record.
